According to documents filed by his family, Ozzy Osbourne‘s cause of death was a heart attack.
The New York Times reports that the singer’s death certificate was filed at a London registry by his daughter Aimee. The document lists cardiac arrest and coronary artery disease among the causes, while also noting that he had Parkinson’s disease.
The certificate says Osbourne died of “(a) Out of hospital cardiac arrest (b) Acute myocardial infarction (c) Coronary artery disease and Parkinson’s disease with autonomic dysfunction (Joint Causes).”
Osbourne died on July 22, just weeks after headlining the star-studded Back to the Beginning farewell concert by performing both solo and with the original Black Sabbath lineup.

The Daily Mail had previously reported that an air ambulance was dispatched to Osbourne’s Buckinghamshire estate at 10:30 that morning, and spent two hours treating the singer before he died surrounded by his family.
The Times goes on to explain that a Thames Valley Air Ambulance service spokesperson said in an email that its team had been “dispatched to provide advanced critical care at an incident near Chalfront St. Giles on July 22,” but offered no further details.
After stopping near Osbourne’s home, the ambulance flew about eight miles southwest to Harefield Hospital in a London suburb named Uxbridge, according to Flightradar24, a company that collects and reports data on aircraft movements. The helicopter spent “about an hour” at the hospital with its engines running, a Flightradar24 spokesman said.
